Purpose of the Transportation Ministry Summit Church provides a free transportation service as part of its outreach ministry to assist individuals with transportation to church services, medical appointments, or grocery shopping. This service is offered by volunteer drivers to support our community with care and compassion.
Consent and Acknowledgment I, the undersigned, acknowledge that I am voluntarily participating in the transportation services provided by Summit Church. I understand that this is a complimentary service offered by volunteers of Summit Church and that no payment or compensation is required for these services.
Assumption of Risk I understand that there are inherent risks associated with transportation, including but not limited to accidents, injuries, or property damage. I voluntarily assume all risks associated with my participation in the transportation services provided by Summit Church.
Liability Waiver and Release In consideration of being permitted to use the transportation services provided by Summit Church, I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby release, waive, discharge, and covenant not to sue Summit Church, its officers, employees, volunteers, or agents from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or my property while participating in the transportation services, whether caused by the negligence of Summit Church, its volunteers, or otherwise.
Indemnification I agree to indemnify and hold harmless Summit Church, its officers, employees, volunteers, and agents from any loss, liability, damage, or costs, including court costs and attorney fees, that they may incur due to my participation in the transportation services.
Medical Consent In the event of an emergency, I authorize Summit Church volunteers to seek and obtain medical treatment on my behalf if I am unable to provide consent at the time. I understand that Summit Church is not responsible for any medical costs incurred.
Acknowledgment of Understanding I have read this Consent and Liability Waiver Form in its entirety and fully understand its terms. I understand that I am giving up substantial rights, including my right to sue Summit Church for any injuries or damages incurred during the transportation services. I sign this form freely and voluntarily without any inducement.
